Write a 3-4 page paper that describes a conflict or disorienting dilemma from your experience that involved health care providers from one or more health care fields (in the retail pharmacy setting). Choose a scenario that relates to all of the questions listed below.

Example 1 scenario: An elderly patient arrives for a Medicare Part D selection visit and medication review. The pharmacist recognizes the patient is falling a lot and has very low blood pressure. The patient is on multiple blood pressure medications. The pharmacist phones the primary care doctor and recommends changes to the blood pressure medication regimen.

Example 2: the doctor prescribes an anti-depressant medication with Trazadone and an opiod which results in serotonin syndrome. The pharmacist contacts the primary care provider on the appropriate direction of use / alternative medication. The pharmacist also counsels the patient to ensure the appropriate method of taking medication and to space the medication at different times of the day.
