Instructions:

Summarize what you know about the Chicago community (Austin, North Lawndale, and West Garfield Park), population, and health problem drawing on the Rush CHNA. Conduct additional research to fill in gaps in your knowledge. Consider the following:

· Do we know all we need to know about this health problem?

· Do we need to know more about the characteristics of people like those in our community?

· Do we need to know more about the causes that were identified in the CHNA to understand better where we might target an intervention?

Search and integrate at least two articles (ideally, scholarly articles but reports and documents from public health organizations are acceptable) and one solution (a program or intervention used to address the problem) relevant to your health problem. You can draw on the research you found in Assignment 4.

The Background should be about 2 pages (500 words) and cover these points:

· Describe the health problem and significance. Introduce the health problem and provide enough background so readers will understand the significance of the health problem in terms of impact and/or outcomes.

· Community, population, setting. Who is affected by this problem? Describe the broader community and population and subgroups or target populations who are most affected or bear the greatest burden. What are the characteristics in terms of relevant demographics, socioeconomic factors, and health outcomes?

· Causes and risk factors. What are the causes and contributing factors in the research literature?

· Evidence-based interventions. What interventions have been tried in the past? What works and what doesn’t work in addressing this health problem?

· Cite your sources (APA style) and include a reference list (APA style) at the end of the document.
