Include the person’s age now, geographic location(s) over lifetime, types and sources of health care received and the estimated costs, how often health care was sought and how it was paid for, significant personal health conditions if the person is willing to share, types of preventive care received like immunizations, and finally the person’s perspectives about quality, cost and access then and now.  Add anything else that you feel is relevant to this assignment.
