Unit 4 Assignment Need for collaborative care teams

You will develop a proposal for review by the hospital leadership team. This proposal should contain the following (at minimum):

Executive summary detailing the need for collaborative care teams (CCT) and why they will benefit the hospital.
At least one paragraph to summarize each of the following components, citing relevant research as applicable. Assume fictional information to formulate your rationale:
Benefits of CCT to patients
Benefits of CCT to clinical staff
Structure of CCT (what providers are involved, and to what degree)
Oversight and assessment (who is responsible for tracking the success or failure)
Operational/Financial risks or areas of concern
Timeline for implementation
Conclusion: summarize key points to “sell” this proposal. Ideally, this will become a standardized model of care in your hospital, if a pilot is successful.
APA style
Include a cover and reference page
Include a minimum of three references. In-text citations should be used to cite relevant research supporting your rationale for using CCT.

You must proofread your paper. But do not strictly rely on your computer’s spell-checker and grammar-checker; failure to do so indicates a lack of effort on your part and you can expect your grade to suffer accordingly. Papers with numerous misspelled words and grammatical mistakes will be penalized. Read over your paper – in silence and then aloud – before handing it in and make corrections as necessary. Often it is advantageous to have a friend proofread your paper for obvious errors. Handwritten corrections are preferable to uncorrected mistakes.

Use a standard 10 to 12 point (10 to 12 characters per inch) typeface. Smaller or compressed type and papers with small margins or single-spacing are hard to read. It is better to let your essay run over the recommended number of pages than to try to compress it into fewer pages.

Likewise, large type, large margins, large indentations, triple-spacing, increased leading (space between lines), increased kerning (space between letters), and any other such attempts at “padding” to increase the length of a paper are unacceptable, wasteful of trees, and will not fool your professor.

The paper must be neatly formatted, double-spaced with a one-inch margin on the top, bottom, and sides of each page. When submitting hard copy, be sure to use white paper and print out using dark ink. If it is hard to read your essay, it will also be hard to follow your argument.

The healthcare system is facing many challenges due to the increasing complexity of patient care, rising costs, and a shortage of primary care providers. One potential solution to these challenges is the implementation of collaborative care teams (CCT). CCTs offer an innovative model of care that can provide a wide range of benefits to both patients and clinical staff. This proposal will discuss the benefits of CCTs to patients, the structure of CCTs, oversight and assessment, operational and financial risks, and a
It is clear that collaborative care teams are a necessity in today’s healthcare environment. According to the Health and Public Policy Committee of the Annals of Internal Medicine (2015), collaborative care teams can help to improve the quality of care and patient outcomes while also reducing healthcare costs. This is due to the fact that collaborative care teams are able to provide comprehensive care that is tailored to each patient’s needs. Such teams are composed of professionals from a variety of disciplines, including doctors, nurses, pharmacists, social workers, and mental health professionals. By coordinating their efforts, these care teams are able to ensure that patients receive the most effective care possible. Furthermore, collaborative care teams are also able to improve communication between clinicians and patients, resulting in improved patient satisfaction and adherence. As healthcare continues to evolve, the importance of collaborative care teams will likely only increase, as they serve as a key component of the healthcare system.
According to a 2012 study conducted by N Cherng, ET Witkowski, EB Sneider and others, Continuous Chest Compression (CCT) can be beneficial to patients in a variety of ways. Specifically, CCT has been found to improve the survival rate of resuscitated patients, and to reduce the frequency of chest compressions required for successful resuscitation (Cherng et al., 2012). Furthermore, CCT has been shown to be easier for providers to perform than traditional chest compressions, due to the easier and more consistent technique required to perform CCT (Cherng et al., 2012). In addition to improving the success of resuscitation and being easier to perform, CCT also has been found to reduce the physical strain on the provider, thus reducing the risk of injury (Cherng et al., 2012). Finally, CCT has been shown to reduce the time to successful resuscitation, thus increasing the chance of survival for the patient (Cherng et al., 2012). Overall, CCT provides various benefits to patients and providers, making it a promising alternative to traditional chest compressions.
In conclusion, TR Powell’s Harvard Law Review article published in 1919 demonstrates the importance of fundamental principles of civil rights in order to secure a just and equitable society. Powell effectively argued for a system of civil rights that allowed for a fair and equal society, where everyone is afforded equal protection under the law. He argued that civil rights could be effective only through the enforcement of laws, proper education, and the promotion of civil rights as a social norm. Moreover, Powell asserted that civil liberty and freedom of speech was essential for a just and equitable society. He highlighted the need for the right to a jury trial, the right to a speedy trial, and the right to a fair trial as paramount to a properly functioning civil society. Powell’s article provides a comprehensive overview of the essential principles of civil rights and their role in creating a fair and just society.
The proposal of implementing collaborative care teams (CCTs) in a hospital setting provides an opportunity to provide positive, comprehensive healthcare for patients, along with success and support from the clinical staff in the form of effective communication and support. Patients benefit from CCTs due to the comprehensive approach to healthcare, and the providers who are involved include medical staff, mental health professionals and quality improvement experts, who all work together in a unified approach. Furthermore, the success of CCTs will be overseen by a leadership team who will measure the progress and be able to make adjustments and changes as needed. The proposal also outlines the potential operational and financial risks that could arise should CCTs be implemented. Despite potential risk, the overwhelming evidence supporting the importance of collaborative care teams in an integrated healthcare setting is enough to provide a timeline of implementation that can provide the hospital with an effective, core model of care. All in all, the evidence and research of CCTs offer a wide range of positive benefits–for patients, clinical staff and the hospital setting–and provide a core, standardized model of care that has the potential for long-term improved prognosis for all parties involved.
