  Data collected from the change proposal will be collected and reviewed over a 6–12-month period

  Benchmarks used to measure this change proposal will include comparing pressure injuries related to medical device within the other three community hospital, HOCC, Midstate and UCONN, using the data collected to implement evidence-based intervention,

  Ensure that the skin assessments and interventions set in place are being charted in an appropriate and timely manner.

  Calculate revenue of money saved and reimbursement received by institution with decrease device related injuries.

  Data collected from the project will be collected and send to the quality control department for analysis and information will be discussed throughout the unit via huddle.

  The desire for the change proposal is to decrease device related pressure injuries in HFS by 50% over a 12-month period.

  Evaluation for feedback from the change proposal to improve the process will be collecting a detail survey from floor staffs involved in the process, create suggestion boxes on unit for staff to utilize about their perspective, safety huddles and unit council meeting, AHO reports, feedback questionnaires for process improvement, HCAP score and employee education module score.
