Compare and contrast the pathophysiology between Alzheimer’s disease and frontotemporal dementia.

  • According to DeTure and Dickson (2019), Alzheimer’s disease is a progressive neurodegenerative disease manifested as deficits in memory and cognition. Alzheimer’s disease is diagnosed pathologically by recognizing features that are highly suggestive of the disease rather than specific combination of features for diagnosis (DeTure and Dickson, 2019).
  • Khan and DeJesus (2021) describe frontotemporal dementia as being caused by nerve cell damage to the frontal and temporal lobes of the brain, resulting in the loss of intellectual functions including memory problems, thinking, reasoning, and functioning; usually with a sever decrease in the patients’ ability to perform ADLs.

Identify the clinical findings from the case that supports a diagnosis of Alzheimer’s disease.

  • The patient becoming lost in the neighborhood he resided in for 35 years.
  • Wandering and being brought home by neighbors
  • Worsening memory
  • Angry and defensive response toward being questioned about his behavior
  • Trouble dressing himself
  • Inability to balance his checkbook
  • Unable to make decisions as he previously had

Explain one hypothesis that explains the development of Alzheimer’s disease.

  • Haam and Yakel (2017) present the cholinergic hypothesis on the development of Alzheimer’s disease. The cholinergic hypothesis states a dysfunction of acetylcholine containing neurons in the brain causes the cognitive decline seen in Alzheimer’s patients (Haam & Yakel, 2017). Elevation of acetylcholine by the blockage of acetylcholinesterase is the most used method in treating Alzheimer’s disease (Haam & Yakel, 2017).

Discuss the patient’s likely stage of Alzheimer’s disease.

  • The patient in this scenario appears to be in the middle stages of Alzheimer’s, which can be the longest stage. This is evidenced by the patients becoming easily lost, forgetfulness, decrease in ADL function, along with being easily agitated and defensive when confronted with conversation about these changes, but is able to maintain independence with guidance and direction
