Scenario

Mackenzie Jones was born on 12/6/98, he was diagnosed with paranoid schizophrenia three years ago. His parents found they could not cope with his delusional behaviour which they found incredibly distressing and they were worried for their own safety. Mackenzie has been compulsorily admitted to hospital on four occasions, but his medication now seems to be maintaining him in a stable state. He has moved and lives in a bed sit at 25A Coldham Street, Bexley.

He is visited daily by a support worker (Ellie) and is also monitored by a Community Psychiatric Nurse, Karl.  Initially Mackenzie seemed to be thriving in his own space. He was going to a boxing club and Gym and had a part time job in a fitness shop.

However, on his last visit, Karl noticed that he was quite agitated. Ellie has introduced Mackenzie to her friends, and they were getting on well. He started a relationship with one of Ellie’s friends, Karen Brocklehurst, 28. Karen is often short of money and he helps her out. Recently, Mackenzie loaned her £500. It was all the savings that he had; his parents had given it to him to help furnish the bedsit. Karen got angry when he asked for the money back; she denied it was a loan and says he gave it to her. When he met up with his new friends at the local pub, like usual he bought the drinks, but when he was unable to buy more drinks, they all started mocking him. Mackenzie felt uncomfortable and angry, but didn’t feel able to say anything. He spoke to Ellie about it, but she shrugged it off saying ‘can’t he take a joke’. Yesterday, he almost got into a fight with his neighbour, who had taken in a parcel for him. Mackenzie thought he was stealing it. He told Karl that he is lonely and wants things to work out with Karen, but he is worried she may be taking advantage of him. He wonders if he can trust anyone.

Guidance on completing the task

Task 1

Ensure you have fully completed the form using the boxes and the free text. If you are unable to complete a box with the information provided. Only use the information provided – do not create your own case study.

Make sure you have clearly addressed the criteria for section 42 of the Care Act 2014 within the form.

Identify the type of abuse which has occurred or is at risk of occurring and who is probably responsible.

Identify who needs to be involved and any immediate action and subsequent actions that are needed.

Identify if the person at risk has mental capacity and if so, have they given informed consent. If they may have difficulty taking part in the process, identify what actions need to be taken.

Identify what the person at risk would like to happen.

Reference to relevant legislation and policy in the approach you take.

Task 2

You may have identified several types of abuse that could be occurring or that is at risk of occurring. Identify which one that you think is the greatest risk in this situation and justify your reasons why. Ensure you draw on the literature and the details of the case study to support your answer.

Suggest which intervention(s) are most likely to work when this type of abuse occurs.

Ensure you explore the difficulties and benefits of using these interventions in this case.

Task 3

Use the mental capacity test, the guiding principles, and the code of practice to the Mental Capacity Act 2005 to explain why you think this person either has or does not have mental capacity.

Given your answer, explain how it would impact on how you worked with this person.

Task 4

Explain how you would work with this person using the making safeguarding personal approach.

Explore the tension between being person-centred and addressing risks, including the implications of your mental capacity assessment.

In addressing these four tasks, ensure you remain consistent in your answers throughout.
