Steps to completing your Wellness Plan:

Complete the Five Factor Wellness Inventory and review your results.
Complete the questions posed in the Personal Report and the Wellness & Habit Change Workbook.
Identify two target areas in which you’d like to set some goals for improving your Wellness. The areas you choose may be areas where you score a bit lower on the Wellness Scales in your Personal Report.
Using SMART Goals, develop one goal for each of your two target areas to be carried out over the next 8 weeks. We realize that this is challenging as you are a new graduate student in counseling – but NOW is the time to make time for yourself. Life does not slow down for us to care for ourselves; we need to make caring for ourselves a priority.

SMART GOALS:

Specific: Detailed and clear

Measurable: Write the goal so that you can measure how well you are achieving your

goal, e.g., methods to track or count behaviors

Aggressive yet Attainable: Develop goals that are challenging. If goals are too easy

accomplish, we generally lose interest in them.

Relevant: The goal you develop needs to directly address the Wellness area in which

you scored lower.

Time Bound: We have an 8 week time frame. Third week of class through the 11^th

week of class. Additionally time parameters such as time of day, amount of

time on activity etc. are also key.

Write a 4-5 page double-spaced paper which outlines this entire process and your reactions to the process. Be sure to include your SMART goals in your Paper, as well as your hopes for changes in your life. Discuss fully how your Wellness plan can assist you in becoming a skilled and ethical counselor. Do not include your Five Factor reports.
